Background: Cognitive behavior therapy (CBT) is a widely used psychotherapy method with empirical research support. In recent years, the localization of CBT related to culture has attracted more and more Chinese scholars' attention. But no scholars have systematically conducted localization research on CBT using idioms as materials. Objective: Screen out the idioms that can reflect the interaction between the two core therapeutic elements (cognition and emotion) of cognitive behavior therapy (CBT)and further determine the idioms that can be used in CBT practice. Methods: The Xinhua Idiom Dictionary (2nd Edition) published by the Commercial Press was used as a tool, and three rounds of manual screening was adopted. The screeners were composed of two graduate students of Clinical psychology (who had received systematic CBT training), a senior CBT therapist (with 10 years of CBT treatment experience) and four CBT experts (CBT trainers and supervisors), and finally determined the idioms that can reflect the interaction between cognition and emotion in CBT. Based on the final determined idioms, a survey questionnaire was prepared and sampled among college students from different majors and universities to calculate their understanding of each idiom. Further identification of idioms that can be used as CBT treatment tools and applied in CBT practice, laying the foundation for localization research of CBT. Results: In Xinhua Idiom Dictionary (2nd Edition), there are 13 idioms that reflect cognition affecting emotion, and 5 idioms that reflect emotion affecting cognition. There are twelve idioms recommended as CBT treatment tools. For example, mistake the shadow of a bow in one's cup as a snake”,also known as beigongsheying and every bush and tree looks like an enemy, also known as caomujiebing. Among them, there are 8 idioms that reflect cognition affecting emotion and 4 idioms that reflect emotion affecting cognition. Conclusion: Idioms can reflect the mutual influence between the two core therapeutic elements (cognition and emotion) of CBT, and can be attempted for CBT treatment practice. Based on these, CBT localization exploration related to Chinese culture can be carried out.
