| 【摘要】:背景 功能锻炼依从性是影响乳腺癌患者术后康复效果的重要因素，患者的症状负担和希望水平均与功能锻炼依从性有关，但在乳腺癌术后患者群体中，三者关系证据有限。目的 探讨乳腺癌术后患者希望水平、症状负担与功能锻炼依从性的关系，为改善乳腺癌术后患者功能锻炼依从性提供参考。方法 于2023年3月26~12月9日，连续选取四川省某所三甲医院的308例乳腺癌术后患者为研究对象。采用中文版安德森症状评估量表（MDASI-C）、中文版Herth希望量表（HHS-C）、乳腺癌术后功能锻炼依从性量表进行调查。采用Process中介效应检验分析希望水平在乳腺癌术后患者症状负担与功能锻炼依从性之间的作用路径。结果 乳腺癌术后患者MDASI-C与HHS-C总评分呈负相关（r=-0.202，P<0.01），MDASI-C与功能锻炼依从性总评分呈负相关（r=-0.279，P<0.01），HHS-C与功能锻炼依从性总评分呈正相关（r=0.453，P<0.01）。希望水平在症状负担与功能锻炼依从性之间具有部分中介作用，中介效应值为-0.083（95% CI：-0.139~-0.009），占总效应的29.74%。结论 乳腺癌术后患者的症状负担可以直接影响功能锻炼依从性，也可以通过希望水平间接影响功能锻炼依从性。 |

| 【Abstract】 Background Functional exercise adherence is an important factor affecting the postoperative rehabilitation effect of breast cancer patients, and the symptom burden and hope level of patients are related to functional exercise adherence, but there is limited evidence of the relationship between the three in breast cancer postoperative patients. Objective To investigate the relationship between the level of hope, symptom burden and compliance with functional exercise and the internal mechanism of breast cancer patients after breast cancer surgery, so as to provide a reference for alleviating the symptom burden and improving the hope level of breast cancer patients. Methods From March 26 to December 9, 2023, 308 patients with breast cancer after surgery in a top-three hospital in Sichuan Province were selected as the study objects. The Chinese version of the Anderson Symptom Assessment Scale (MDASI-C), the Chinese version of the Herth Hope Scale (HHS-C), and the Functional Exercise Adherence Scale after breast cancer surgery were used for investigation. The Process mediating effect test was used to analyze the mediating effect of hope level on the relationship between symptom burden and functional exercise compliance in breast cancer patients. Results MDASI-C was negatively correlated with HHS-C total score (r=-0.202, P<0.01), MDASI-C was negatively correlated with functional exercise adherence score (r=-0.279, P<0.01), and HHS-C was positively correlated with functional exercise adherence score (r=0.453, P<0.01). Hope level has a partial mediating effect between symptom burden and functional exercise compliance，the mediating effect size was -0.083 (95% CI: -0.139~-0.009), accounting for 29.74% of the total effect. Conclusion The symptom burden of breast cancer patients after surgery can directly affect the adherence of functional exercise, and can also indirectly affect the adherence of functional exercise through the level of hope. |
