The relationship between fear of disease progression and disease perception in patients with chronic obstructive pulmonary disease: the mediating role of social support

      Background Fear of progression (FoP) is a typical psychological state among patients with chronic obstructive pulmonary disease (COPD). The level of FoP and disease perception in these patients is related to the degree of social support they receive. However, there is limited evidence regarding the relationship between these three factors among COPD patients. Objective To explore the mediating role of social support in the relationship between FoP and disease perception among COPD patients, providing reference for targeted nursing interventions to promote their mental health and disease recovery. Methods From March 9th to August 1st, 2024, a total of 412 COPD patients from a tertiary hospital in Sichuan Province were consecutively recruited as study participants. The Chinese version of the Fear of Progression Questionnaire-Short Form (FoP-Q-SF), the Brief Illness Perception Questionnaire (BIPQ) in Chinese, and the Social Support Scale (SSS) were used for data collection. Pearson correlation analysis was employed to examine the correlations among the scores of these scales. Model 4 in the Process macro program of SPSS 25.0 was utilized to analyze the mediating role of social support in the relationship between FoP and disease perception. Results The disease perception score of COPD patients was positively correlated with the fear of disease progression score (r=0.238, P<0.01), negatively correlated with the social support score (r=-0.26, P<0.01), and the disease perception score was negatively correlated with the social support score (r=-0.135, P<0.01). Social support may be the pathway between disease perception and fear of disease progression, with an effect size of 0.167 (95% CI: 0.096-0.237), accounting for 86.97% of the total effect. Conclusion FoP in COPD patients can directly influence their disease perception and indirectly through social support. Social support serves as a mediating pathway between FoP and disease perception, highlighting its importance in managing the psychological well-being and disease recovery of COPD patients
