      背景 家庭干预是自闭症患儿干预的重要组成部分,而既往关于以家庭为主导的密集行为干预对自闭症幼儿语言能力影响的研究不足。目的 研究以语言行为里程碑评估及安置程序(VB-MAPP)为基础的家庭密集行为干预对自闭症谱系障碍(ASD)幼儿语言能力的影响,为制定ASD患儿的家庭干预策略提供参考。方法 选取2020年9月-2021年12月在汕头大学医学院第二附属医院儿童康复科就诊、符合《精神障碍诊断与统计手册(第5版)》(DSM-5)ASD诊断标准的2~3岁患儿作为研究对象。采用同期非随机对照试验,将患儿分为研究组(n=24)和对照组(n=14),研究组接受以VB-MAPP为基础的家庭密集行为干预,对照组接受其他特殊机构或早教机构培训,两组干预均持续6个月。干预前后,对两组患儿进行VB-MAPP里程碑评估,选取其中的“提要求、命名和听者反应”作为研究指标。采用t检验或秩和检验比较研究组与对照组干预效果差异,采用多元线性回归分析考查影响干预效果的因素。结果 干预后,研究组VB-MAPP总评分、提要求、命名和听者反应评分均高于对照组(Z=-4.339~-2.195,P均<0.05);多元线性回归分析结果显示,前三个月平均每周家庭训练时长对听者反应能力有影响(β=6.065,t=2.170,95% CI:0.339~11.791,P<0.05)。结论 以VB-MAPP为基础的早期家庭密集行为干预对ASD幼儿在提要求、命名、听者反应的语言能力发育方面具有良好效果。

      Background Family intervention is an important part in the treatment of children with autism, but previous studies about effect of parent-mediated family intensive behavioral intervention on language ability in children with ASD were limited. Objective To provide reference for formulating family intervention strategies for children with ASD, our study explored the effect of family intensive behavioral intervention based on Verbal Bahaviors Milestones Assessment and Placement Program (VB-MAPP)on the language ability in children with autism. Methods Children aged 2-3 years old who were admitted to the department of children rehabilitation in the Second Affiliated Hospital of Shantou University Medical College from September 2020 to December 2021. All children met the diagnostic criteria for ASD in the Diagnostic and Statistical Manual of Mental Disorders (5th Edition) (DSM-5). Our study is a non-randomized controlled trial, and all children were departed into study group (n=24) and control group (n=14). The study group take part in the VB-MAPP based family intensive behavioral intervention, and the control group participated in other special institutions or early education institutions training, the intervention of the two groups both lasted for 6 months. The VB-MAPP milestone assessment was carried out in the two groups, and the “Mand, Tact and Listener Responds” were selected. Results After intervention, the VB-MAPP milestone score, Mand, Tact and Listener Responds scores in the study group were higher than those in the control group (Z=-4.339~-2.195, P<0.05). Multiple linear regression analysis showed that the average weekly family training time in the previous three months had effect on the ability of Listener Responds (β=6.065, t=2.170, 95%CI=0.339~11.791, P<0.05). Conclusion VB-MAPP-based early intensive family behavior intervention has an advantage effect on the language development of ASD children in Mand, Ttact and Listener Responds.
