| 目的 探讨氟西汀联合舒肝解郁胶囊治疗抑郁症患者的早期临床疗效；比较氟西汀联合疏肝解郁胶囊与单用氟西汀在心率变异性影响上的差异及药物副反应,为抑郁症患者的临床用药提供参考。方法 采用随机数字表法将符合入组条件的,并于2015年12月至2016年6月在新乡医学院第二附属医院门诊就诊和住院的64例抑郁症患者分为联合用药组和氟西汀组,每组32例。氟西汀组患者单独使用氟西汀治疗；联合用药组在氟西汀治疗基础上加服舒肝解郁胶囊。两组患者均于治疗开始前进行24项汉密尔顿抑郁量表(HAMD24)评定、14项汉密尔顿焦虑量表(HAMA14)评定及心率变异性(heart rate variability, HRV)分析,并于治疗第2、4、6周后进行HAMD24及副反应量表(TESS)评定,并在治疗后再次对两组患者进行HRV分析。结果 两组治疗2、4、6周后与治疗前相比,HAMD评分均有所下降,联合用药组HAMD评分较氟西汀组下降更明显；治疗6周后与治疗前相比,两组HRV分析各指标均有所下降；两组治疗后疗效分级差异显著(P＜0.05)；两组副反应比较,差异无显著性(P＞0.05)。结论 舒肝解郁胶囊联合氟西汀治疗抑郁症早期临床疗效确切,效果优于单用氟西汀,能有效缩短起效时间,且不增加不良反应。 |

| Objective To investigate the early clinical efficacy of fluoxetine combined with shuganjieyu capsule in the treatment of depression patients. The differences in heart rate variability and side effects between fluoxetine combined with shugan jieyu capsule and fluoxetine alone were compared, so as to provide reference for the clinical use of fluoxetine in patients with depression. Methods 64 patients with depression who were admitted to the second affiliated hospital of xinxiang medical college from December 2015 to June 2016 were divided into the combination drug group and the fluoxetine group by using the random number table method, with 32 patients in each group. Patients in the fluoxetine group were treated with fluoxetine alone.The combined group was treated with fluoxetine plus shugan jieyu capsule. Two groups of patients before the start of treatment to 24 Hamilton depression scale (HAMD24), 14 Hamilton anxiety scale (HAMA14) and heart rate variability (heart rate variability, HRV) analysis, and in the treatment of 2, 4, 6 weeks after HAMD24 symptoms scale (TESS) and assess, and once again after treatment on two groups of patients with HRV analysis. Results The HAMD scores of the two groups decreased after 2, 4 and 6 weeks of treatment as compared with those before treatment. The HAMD score of the combination group was decreased more significantly than that of the fluoxetine group. After six weeks of treatment, compared with those before treatment, each index of HRV analysis in the two groups was decreased. There was a significant difference in efficacy grades between the two groups after treatment (P < 0.05). There was no significant difference in adverse reactions between the two groups (P > 0.05). Conclusion shugan jieyu capsule combined with fluoxetine has a definite clinical effect in the early stage of the treatment of depression. The effect is better than that of fluoxetine alone, which can effectively shorten the onset time without increasing adverse reactions. |
