Background Stroke patients commonly experience varying degrees of psychological distress. Existing studies have shown that the psychological distress of patients is related to their coping style and resourcefulness. However, the potential pathways among the three are still unclear. Objective Exploring the mediating role of resourcefulness between coping styles and psychological distress and providing a reference for the intervention of psychological distress in stroke patients. Methods From January to May 2023, 350 patients who met the diagnostic criteria for stroke according to the Diagnostic Criteria of Cerebrovascular Diseases in China (version 2019) and were admitted to the neurology departments of two tertiary grade-A hospitals in Luzhou City were selected as the research subjects. A general information questionnaire was used to collect general information, and the Resourcefulness Scale (RS), Medical Coping Mode Questionnaire (MCMQ) and Distress Thermometer (DT) were used for investigation. Pearson correlation analysis was used to examine the correlation between the scores of each scale, Model 4 of the Process v3.4.1 program was used to analyze the path of resourcefulness between coping styles and psychological distress, and the Bootstrap method was used to test the mediating effect. Results The DT score was negatively correlated with the confrontation and RS scores (r=-0.581, -0.716, P both <0.01), and positively correlated with the avoidance and resignation scores (r=0.432, 0.758, P both <0.01). The score of confrontation was positively correlated with the RS score (r=0.700, P<0.01), and the avoidance and resignation scores were negatively correlated with the RS score (r=-0.460, -0.566, both P<0.01); Resourcefulness fully mediated the relationship between confrontation and psychological distress (95% CI: -0.440~-0.283). It also partially mediated the relationships between avoidance and psychological distress, and between resignation and psychological distress, with mediation effect proportions of 62.65% (95% CI: 0.147~0.267) and 27.07% (95% CI: 0.137~0.236) respectively. Conclusion The coping styles of stroke patients can directly affect their psychological distress, and can also indirectly influence their psychological distress through resourcefulness. Resourcefulness may be the path between coping style and psychological distress.
